Several ideas, one piece.
Here the work isn't in the elements, it's in the order: what overlaps what, where the eye enters, how much air is left between things so the whole reads at once instead of as a pile. They almost always begin with someone who arrives with four ideas and won't drop any of them. And they get drawn onto the actual limb rather than onto flat paper — a forearm is seen in parts as the arm turns, so the arrangement has to close from three different angles.
